Joint Care Physiatry is a physician-owned and physician-led Physical Medicine & Rehabilitation (Physiatry) practice focused on restoring function, mobility, and quality of life for patients across the full continuum of care.

Physiatrists are medical doctors who specialize in diagnosing and treating conditions affecting movement, strength, and physical function, without surgery. We manage pain, mobility limitations, neurologic and musculoskeletal conditions, and the complex rehabilitation needs that often follow hospitalization.

What makes Joint Care Physiatry unique is our integrated inpatient and post-discharge model. We provide hands-on, in-person care in Inpatient Rehabilitation Facilities (IRFs) and Skilled Nursing Facilities (SNFs), then seamlessly continue care after discharge through telehealth follow-up visits. These virtual visits ensure patients receive timely evaluations, accurate documentation, and rapid coordination for essential Durable Medical Equipment (DME) such as wheelchairs, prosthetics, and mobility devices.

By staying involved beyond the facility walls, we close gaps in care, reduce delays, and help patients transition safely from hospital to home—fully supported, properly equipped, and set up for long-term functional success.
